Anti-Neutrophil Cytoplasmic Antibodies (ANCA) are detected by immunofluorescence assays in more than 95% of vasculitis patients. ANCA are autoantibodies that react with neutrophil specific antigens, most frequently myeloperoxidase (MPO) and proteinase-3 (PR3).

On ethanol-fixed neutrophils two prominent patterns of ANCA can be identified, a diffuse, granular cytoplasmic staining (C-ANCA) and a perinuclear staining with or without nuclear involvement (P-ANCA). Less frequently, atypical ANCA patterns not fitting the descriptions of C and P-ANCA can be recognised.

Using a combination of ethanol-fixed neutrophils and formalin-fixed neutrophils can help to distinguish non-ANCA artefacts from true P-ANCA and C-ANCA staining patterns.
